To obtain a copy of a Death Certificate, we will need to have a Death Certificate Application form completed by the appropriate individual.
Once you have completed the Death Certificate Application, we will need you to come into the Town Office to receive a formal copy of the Death Certificate.
- 1st copy of Death Certificate - $15
- 2nd copy of Death Certificate - $6 (If within same request for 1st copy)

- Photo ID
